Insurance Coverage Information
Active Life is contracted with most federal, state and private insurance companies and payors, and also works with many managed care organizations, medical groups and worker's compensation benefit administrators. While it is ultimately the client's responsibility to understand his or her own benefits and insurance coverage for prescribed services at Active Life, we understand the overall process can be confusing and create concern or fear of being responsible for costs that were unanticipated. Active Life is dedicated to helping simplify this process and with creating as much transparency as possible with any expected or estimated costs associated with services or products provided at one of our clinics. To protect our clients, Active Life will verify benefits and coverage with a client's health plan in advance of providing services when possible, including confirmation of products or services being a covered benefit of a client's plan, and any costs related to deductibles, co-pays or any other related out-of-pocket expenses. In most cases, clients will be provided a financial responsibility form in advance of delivery of a service or product that provides a detailed estimate of the total cost for services, and a breakdown of charges expected to be paid by the client's insurance, and by the client directly.

Additionally, authorizations received from any managed care organization, medical group or similar is not a guarantee of payment. Though it is not common, there are some cases where insurance companies or plans deny coverage or the billing claim, in part or in whole, and in these cases clients are responsible for any charges assigned by their insurance plan to the client even if the amount is different than Active Life's initial estimate.
